This is the privacy policy, as updated from time to time (this “Privacy Policy”), of UberDoc, Inc. (“We” “Us” or “Uberdoc”). You agree to be bound by this Privacy Policy. Capitalized terms used but not defined in this Privacy Policy have the meanings specified in the UberDoc Terms of Use [https://uber-docs.com/terms-conditions]. We respect your privacy and are committed to protecting your personal information. We will comply with this Privacy Policy and applicable U.S. privacy law when we gather information from users of the Services (as defined in the Terms of Use). BY USING THE SERVICES, YOU AGREE TO THE PRACTICES AND POLICIES OUTLINED IN THIS PRIVACY POLICY AND CONSENT TO THE COLLECTION, USE, AND SHARING OF YOUR INFORMATION AS DESCRIBED HEREIN.

If you do not agree with this Privacy Policy, you may not use the Services.

If you use the Services on behalf of another person or entity, you represent that you are authorized to accept this Privacy Policy on their behalf.

1. INFORMATION WE COLLECT

1.1 Personal Information

Some Services require Personal Information to function properly. This may include:

  • Contact data (email address, phone number)
  • Demographic data (gender, date of birth, ZIP code)
  • Insurance data (carrier, plan, member ID, group ID, payer ID)
  • Medical data, including:
    • Healthcare Providers visited or of interest
    • Reasons and dates of visits
    • Medical history and health information you choose to share
  • Payment information
  • Other voluntarily provided information, including:
    • SSN
    • Passwords or unique identifiers
    • Information in emails or letters sent to us
  • Healthcare Provider information submitted on their behalf (contact info, demographics, licenses)

You may still use limited Services without providing Personal Information, but some features will be unavailable.

1.2 Traffic Data

We may automatically collect:

  • IP address
  • Domain server
  • Device type
  • Browser type
  • Referring webpage
  • Geolocation data
  • Usage statistics

Some Traffic Data may be considered Personal Information under applicable law.

We may collect additional information as disclosed at the point of collection or with your consent.

2. HIPAA AND PHI

2.1 Protected Health Information (PHI)

Under HIPAA, certain health-related data collected by UberDoc may qualify as Protected Health Information (PHI).

UberDoc uses and discloses PHI only as permitted by:

  • Your Healthcare Provider(s)
  • HIPAA regulations

2.2 UberDoc Authorization

You may be asked to e-sign the UberDoc User Authorization.
Signing is voluntary.

  • If there is a conflict between this Privacy Policy and the Authorization, the Authorization governs PHI use.
  • Without Authorization, PHI is used only as allowed by HIPAA and your providers.

3. HOW WE COLLECT INFORMATION

We collect information when you:

  • Search for Healthcare Providers or appointments
  • Complete Medical History Forms
  • Enter information in text fields, surveys, or questionnaires
  • Download applications or software
  • Use our mobile app (including location data)
  • Interact with cookies and tracking technologies
  • Contact us directly
  • Submit Posted Information (as defined in Terms of Use)

4. TRACKING TOOLS AND “DO NOT TRACK”

4.1 Tracking Tools We Use

  • Cookies – store preferences, login status, analytics, fraud prevention
  • Interest-based advertising cookies
  • Web Beacons (pixel tags)
  • Website analytics services
  • Mobile device identifiers

4.2 Opting Out

You may opt out via:

  • Browser settings
  • Mobile device settings
  • NAI opt-out page
  • DAA consumer opt-out page

Opting out does not stop all ads or data collection.

4.3 Do Not Track Signals

Currently, there is no industry standard for DNT signals.
UberDoc does not respond to DNT signals at this time.

5. USE OF INFORMATION

We use your information to:

  • Provide and manage Services
  • Communicate with you
  • Customize user experience
  • Send reminders and notifications
  • Conduct analytics and research
  • Detect fraud and security issues
  • Combine data from third parties
  • Send marketing communications

De-identified and aggregated data may be used at our discretion.

6. DISCLOSURE OF INFORMATION

We may share information with:

  • Healthcare Providers
  • Emergency services
  • Insurance providers
  • Business partners and service providers
  • Affiliates in mergers or acquisitions
  • Legal authorities, when required

We do not sell email addresses.

7. PUBLIC INFORMATION

Any content you post publicly is not private and may be collected or used by third parties.

8. STORAGE AND SECURITY

We use industry-standard security measures, including encryption.

However, no system is 100% secure. If a breach occurs, we will notify you as required by law.

9. CONTROLLING YOUR INFORMATION

  • Update your account via login
  • Request account deletion at privacy@UberDoc.com
  • Some information may be retained for legal purposes

10. INFORMATION ABOUT CHILDREN

  • Services are not intended for children
  • No knowing collection from children under 13 without parental consent
  • Minors may use Services only with parental supervision

11. LINKS TO OTHER WEBSITES

UberDoc is not responsible for third-party websites or their privacy practices.

12. INTERNATIONAL USERS

Services are governed by U.S. law.
By using them, you consent to data transfer to the United States.

13. UPDATES TO THIS PRIVACY POLICY

We may update this policy at any time.
Continued use of Services indicates acceptance of changes.
